2. The information provided in the "Giveaway Registration" portion of the form is NOT associated with the provided Anonymous Health Information.
3. After the close of the Assessment Period, THREE individuals are chosen by a random generator to receive a Samsung 55" 4k TV.
4. If you do not wish to be part of the Giveaway, simply do not complete the "Giveaway Registration" portion of the form.
ANONYMOUS HEALTH INFORMATION:
1. Any and all individual health information is considered confidential, and NOT shared with your employer.
2. The purpose of this Assessment is to provide general demographic information for the development of wellness programs.
3. Information provided in the "Registration Giveaway" portion of the form, such as First Name, Last Name, etc. is NOT linked to the Anonymous Health Information provided.
